IF YOU ARE A STUDENT, PLEASE SEND:

A copy of your School ID card with ALL of the following:

  • Your Picture
  • School Name
  • CURRENT Semester or CURRENT School Year on the card

    OR

A copy of your Photo ID card (School ID, Driver's License, Passport) AND ONE of the following:

  • Copy of CURRENT class schedule
  • Copy of receipt showing payment for CURRENT semester class
  • Letter from instructor or school on school letterhead verifying CURRENT enrollment

IF YOU ARE FACULTY/STAFF, PLEASE SEND:

A copy of your School ID card with ALL of the following:

  • Your Picture
  • School Name
  • CURRENT Semester or CURRENT School Year on the card

    OR

A copy of your Photo ID card (School ID, Driver's License, or Passport) and ONE of the following:

  • Copy of RECENT pay stub (within 3 months)
  • Copy of CURRENT teaching contract
  • Letter from department head or supervisor on school letterhead verifying CURRENT school employment
