Guidelines & Form for Completing Immunization Requirements MSN Students click here for the Graduate School Immunization Form **The following additional requirements are for Nursing Students only: **All Students: North Carolina statute G.S. 130A-155.1: Persons attending a college or university, whether public, private or religious, are required to present a Certificate of Immunization or a record of immunization from a high school located in North Carolina. For out-of-state students, a Certificate of Immunization or high school immunization record indicating that the person has received immunizations is required to attend a college or university in North Carolina. The statute applies to all students except the following: If at anytime the above student status changes to: on-campus courses, course load of more than four (4) credit hours, on-campus residence, and/or enrollment in day-time courses, a Certificate of Immunization or record of immunization must be presented on or before the date the person first registers for a quarter or semester. If after July 1, 1986, the exempt student transfers, interrupts study for six (6) months or more or graduates, the statute is applicable upon re-entry to a college or university. Footnote 1: DTP (Diphtheria, Tetanus, Pertussis), DTaP (Diphtheria, Tetanus, acellular Pertussis), Td (Tetanus, Diphtheria): 3 doses of tetanus/diphtheria toxoid of which one must have been within the last 10 years. Footnote 2: Measles : Measles vaccine is not required if any of the following occur: Diagnosis of disease prior to January 1, 1994; Born prior to 1957; Enrolled in college or university for the first time before July 1, 1994; An individual who has been documented by serological testing to have a protective antibody titer against measles. Footnote 3: Mumps: Mumps vaccine is not required if any of the following occur: Born prior to 1957; Enrolled in college or university before July 1, 1994; An individual who has been documented by serological testing to have a protective antibody titer against mumps. Footnote 4: Rubella: Rubella vaccine is not required if any of the following occur: 50 years of age or older; Enrolled in college or university before February 1, 1989 and after their 30th birthday; An individual who has been documented by serological testing to have a protective antibody titer against rubella. Footnote 5: Hepatitis B: Hepatitis B vaccine is not required if you were born before July 1, 1994. Footnote 6: Polio: Only required for students 17 years of age or younger. *** Meningococcal (Meningitis) Disease and Vaccination Information*** Meningococcal disease is a potentially life-threatening bacterial infection. Since 1991, cases of meningococcal disease among 15-24 year olds have increased. Studies show that students residing in residence halls, especially freshmen, appear to be at an increased risk for meningococcal disease. Studies suggest that up to 80% of college cases are vaccine preventable. The vaccine is 85-100% effective in preventing the disease in older children and adults.
